Welm Health LLC (“Welm”) respects your privacy and is committed to protecting your information. This Policy describes how we collect, use, and share information about you.

We may collect the following categories of information:

- Health information (medical history, symptoms, questionnaire responses)
- Payment information (processed through third-party payment processors)

2. Tracking Technologies
Welm may use cookies, Google Analytics, and Google Ads tracking technologies to understand general website usage and measure advertising effectiveness. Meta Pixel is used solely on non-clinical, pre-intake marketing pages such as the homepage and general informational pages. Meta Pixel is not deployed on any page where patients enter health information, complete intake questionnaires, or access their account or care portal. Welm does not transmit Protected Health Information or individually identifiable health information to Meta.

3. How We Use Information
Information may be used for:

- MD Integrations Providers for care delivery
- Licensed pharmacy partners involved in prescription fulfillment

- Analytics vendors, consistent with the Tracking Technologies section above

5. HIPAA
Protected Health Information (“PHI”) is handled pursuant to applicable HIPAA obligations and Welm’s HIPAA Notice of Privacy Practices. Rights related to PHI are governed by HIPAA and the HIPAA Notice, not by this Privacy Policy.
6. Marketing Communications
Users may opt out of marketing emails at any time by clicking the unsubscribe link in any marketing email. Users may opt out of SMS marketing communications by replying STOP to any marketing text message.
7. State Privacy Rights
Depending on the state in which you reside, you may have the following rights with respect to your personal information:
Right to Know / Access.
You may request a copy of the personal information Welm holds about you and information about how it is collected, used, and shared.
Right to Correction.
You may request that Welm correct inaccurate personal information we maintain about you.
Right to Deletion.
You may request that Welm delete personal information we have collected from you, subject to certain legal exceptions including HIPAA retention requirements for Protected Health Information.
Right to Opt Out of Sale or Sharing.
Welm does not sell personal information. Welm does not share personal information for cross-context behavioral advertising purposes, except as disclosed in the Tracking Technologies section above.
Right to Non-Discrimination.
Welm will not discriminate against you for exercising any privacy rights available to you under applicable law.
To submit a privacy rights request, contact [email protected] with the subject line “Privacy Rights Request.” We will respond within forty-five (45) days from receipt. We may request verification of your identity before processing your request.
8. Data Retention
Welm retains personal information for as long as necessary to fulfill the purposes described in this Policy and as required by applicable law, including HIPAA medical record retention obligations.
9. Do Not Track
Welm does not currently respond to Do Not Track signals from browsers.
10. Data Security
Welm uses commercially reasonable administrative, technical, and physical safeguards to protect your information. No method of transmission or storage is completely secure, and Welm cannot guarantee absolute security.
