Welm Health LLC (“Welm”) respects your privacy and is committed to protecting your information. This Policy describes how we collect, use, and share information about you.
1. Information We Collect
We may collect the following categories of information:
- Contact information (name, email, phone, address)
- Health information (medical history, symptoms, questionnaire responses)
- Payment information (processed through third-party payment processors)
- Device and browser data
- IP address and analytics data
2. Tracking Technologies
Welm uses the following tools on welmhealth.com. Google and Meta’s tools are loaded through Google Tag Manager.
- Google Analytics and Google Ads. These record which pages of welmhealth.com are visited, including treatment and product pages; how visitors arrived; and three shopping steps: viewing a plan, adding it to the cart, and starting checkout. The shopping steps carry no amount and never the name of a treatment. These tools do not run on account or patient portal pages, on the Help me choose questionnaire, or on the intake questionnaire.
- Meta Pixel. Runs only on marketing and product information pages. It does not run on the treatment start page (/begin/), the cart or checkout, account or patient portal pages, the Help me choose questionnaire, or the intake questionnaire. When an order is placed, Welm’s server tells Meta that a purchase occurred and its amount. This is linked only to the identifier Meta’s own cookie placed in your browser, which exists only if you arrived from Meta or interacted with Meta on our site. Welm never sends Meta your name, email address, phone number, or what you ordered.
- Microsoft Clarity. Records how pages are used, such as clicks and scrolling, and only if you select Allow. Text you type is masked. Clarity never runs on the cart, checkout, /begin/, account or patient portal pages, or on either questionnaire.
Your choice. On your first visit, a banner asks whether we may measure your visit.
- No thanks turns off Google Analytics, Google Ads, Meta Pixel and Microsoft Clarity in that browser for 180 days.
- Allow permits all four.
- Until you choose, Google Analytics, Google Ads and Meta Pixel run as described above. Clarity waits for Allow.
- If your browser sends a Global Privacy Control or Do Not Track signal, Welm treats it as No thanks automatically.
To change your choice at any time, use “Visit measurement” at the bottom of any page, or contact [email protected].
Welm does not transmit Protected Health Information to Google, Meta or Microsoft.
2A. Questionnaires and Intake Answers
- Help me choose. This questionnaire, at welmhealth.com/bundles/help-me-choose/, suggests a starting protocol. Your answers stay in your browser and are not sent to Welm or anyone else. The suggestion is passed to the product page within your browser. That page loads no analytics, advertising or recording tools.
- Intake questionnaire (intake.welmhealth.com). This loads no analytics, advertising or recording tools. While you fill it in, your progress is saved in your browser on that device, so you can return to it. Photos and files are not saved there. The saved progress is removed when you submit the intake, or after 7 days. When you submit, your answers go to MD Integrations’ licensed providers for review. To make sure they arrive, Welm keeps an encrypted copy on its own servers. That copy is deleted as soon as the provider’s system confirms it has your answers, which is typically within an hour, and in every case within 30 days. Refill check-ins are handled the same way.
3. How We Use Information
Information may be used for:
- Care coordination and prescription fulfillment
- Customer support and communications
- Marketing communications (you may opt out at any time)
- Analytics and platform improvements
- Legal compliance
4. Information Sharing
Information may be shared with:
- MD Integrations Providers for care delivery
- Licensed pharmacy partners involved in prescription fulfillment
- Payment processors
- Google, Meta and Microsoft, only as described in Section 2
- Legal authorities where required by law
5. HIPAA
Protected Health Information (“PHI”) is handled pursuant to applicable HIPAA obligations and Welm’s HIPAA Notice of Privacy Practices. Rights related to PHI are governed by HIPAA and the HIPAA Notice, not by this Privacy Policy.
6. Marketing Communications
Users may opt out of marketing emails at any time by clicking the unsubscribe link in any marketing email. Users may opt out of SMS marketing communications by replying STOP to any marketing text message.
7. State Privacy Rights
Depending on the state in which you reside, you may have the following rights with respect to your personal information:
Right to Know / Access.
You may request a copy of the personal information Welm holds about you and information about how it is collected, used, and shared.
Right to Correction.
You may request that Welm correct inaccurate personal information we maintain about you.
Right to Deletion.
You may request that Welm delete personal information we have collected from you, subject to certain legal exceptions including HIPAA retention requirements for Protected Health Information.
Right to Opt Out of Sale or Sharing.
Welm does not sell personal information. Welm does not share personal information for cross-context behavioral advertising purposes, except as disclosed in the Tracking Technologies section above.
Right to Non-Discrimination.
Welm will not discriminate against you for exercising any privacy rights available to you under applicable law.
To submit a privacy rights request, contact [email protected] with the subject line “Privacy Rights Request.” We will respond within forty-five (45) days from receipt. We may request verification of your identity before processing your request.
8. Data Retention
Welm retains personal information for as long as necessary to fulfill the purposes described in this Policy and as required by applicable law, including HIPAA medical record retention obligations. Intake and refill answers kept to ensure delivery to your provider are deleted as described in Section 2A.
9. Browser Privacy Signals
Welm honors Global Privacy Control and Do Not Track signals. When your browser sends either one, Welm turns off Google Analytics, Google Ads, Meta Pixel and Microsoft Clarity for that browser, as if you had selected No thanks.
10. Data Security
Welm uses commercially reasonable administrative, technical, and physical safeguards to protect your information. No method of transmission or storage is completely secure, and Welm cannot guarantee absolute security.
